• ADM 146 - At Will Employee Notification: You are required to read these policies and successfully complete the online trainings. You will print and submit any acknowledgements or certificates to your supervisor ONLY. DO NOT submit acknowledgements or certificates to Financial Aid Services, Human Resources or 伊人直播 Payroll.

  • Preventing Sexual Harassment: You will be notified by Human Resources to complete this training.
  • Preventing Employee Discrimination: You will be notified by Human Resources to complete this training.
• W-4: Use the Personal Allowances Worksheet to answer question 5.
• I-9: Complete page 7 ONLY. Print and submit pages 7 and 8 ONLY.
• ADM 045 - Bank Direct Deposit: Original Accepted Photo ID/SSN Card will only be accepted and made copies by the Supervisor or Financial Aid Representative Only.

Follow the steps below to complete and submit your State Aid Evaluation Form through the WEBDMC Portal.

1. Log in to the WEBDMC Portal.
2. Select Financial Information.
3. Click Financial Aid.
4. Select Required Documents.
5. Choose the current Academic Year.
6. Click State Aid Evaluation Form.
7. Complete all required fields on the form and submit it.
8. Check your email for the completed form.
9. Open the email containing the form.
10. Download and save the form to your computer or device.
11. Return to the WEBDMC Portal.
12. Navigate back to Required Documents.
13. Click the Manage button next to the State Aid Evaluation Form requirement.
14. Upload the saved form from your device.
15. Verify that the document status changes to Pending Review. If you see Pending Review, your form has been successfully submitted for processing.

Note: If you do not receive the email with your completed form, check your spam or junk folder before contacting the Financial Aid Office.
